- include your email address is the space provided
- if you need a CD, review the Schedule of Charges for shipping fees
- read the license agreement carefully
- sign the form
When ordering any database products, you will receive instructions showing how to log on to our customer download area within 24 hours.
Please note that payment for all data conversion or consulting services require a company or personal check.
